What is an HMR?
- An HMR-accredited pharmacist will come to your home and check that your medications are safe and effective. This is free of charge if you are taking 5 or more medications.
- They will evaluate whether any medications need to be changed for any reason.
- The review includes checking that medications are being taken correctly, stored properly, and have not passed their use-by date.
- The pharmacist also assesses any interactions between the medications. They then provide their findings to your GP.
