How it works

Identifying eligible patients

Cubiko software identifies eligible patients for HMRs each day.

Chemist2U patient recall

Chemist2U sends an SMS to eligible patients, who then book a consult with their GP.

Consults

The GP refers the patient for an HMR as appropriate. Referrals are sent to Chemist2U via HealthLink.

Pharmacist allocation and HMR

Chemist2U assigns an HMR pharmacist who contacts the patient to schedule and conduct a comprehensive HMR in the patient’s home.

HMR report sent to GP

A detailed report is written and sent to the referring GP via HealthLink.

Medication plan and item number claim

The GP collaborates with the patient to create a medication plan and can claim item number 900 as per the Medicare Benefits Schedule (MBS) requirements.

 

 

What happens in an HMR? 

  1. An HMR pharmacist will ensure the patient’s medications are safe and effective. 
  2. They will evaluate whether any medications need to be changed for any reason. 
  3. The review includes checking that medications are being taken correctly, stored properly, and have not passed their use-by date.  
  4. The pharmacist also assesses any interactions between the medications. They then provide their findings to the GP.
