What happens in an HMR? 

  1. An HMR pharmacist will assess the patient’s medications to ensure they are safe, effective, and not causing any harmful interactions.
  2. They will evaluate whether any medications need to be changed for any reason. 
  3. The review includes checking that medications are being taken correctly, stored properly, and have not passed their use-by date.  
  4. The pharmacist then shares their findings with the GP, including an overview of the patient’s medication regimen and their capacity to manage medications.

The right side of your screen will have an Inca pop-up with Domiciliary Medication Management Review (DMMR)’ if the patient is eligible. Click ‘Start’.

Tick the 'Patient consent' box

If the patient provides consent for a referral, tick the box near the top named ‘Patient consent’.

Add 'Chemist2U' as the provider

Click ‘Add provider’ and then click ‘Chemist2U’ in the drop-down menu.

Finalise

Click the ‘Finalise’ button at the bottom. It’s that easy!
